This Notice of Privacy Practices explains how Medtriq may use and disclose your health information and how you can access this information. Please review it carefully.
This notice applies to health information Medtriq creates or receives while providing care, coordinating care, billing for services, operating the practice, and communicating with patients.
Our responsibilities
Medtriq is required by law to protect the privacy and security of protected health information, provide this notice, follow the duties and privacy practices described in this notice, and notify affected individuals if a breach occurs that may have compromised unsecured protected health information.
Medtriq will not use or share your health information other than as described here unless you give written permission. You may revoke that permission in writing unless Medtriq has already relied on it.
How Medtriq may use and share health information
Medtriq may use and share health information for treatment, payment, and health care operations.
Treatment
Medtriq may use and share health information to provide, coordinate, or manage your care. This may include sharing information among Medtriq providers and staff involved in your care, communicating with pharmacies, coordinating referrals, or communicating with other health professionals when appropriate.
Payment
Medtriq may use and share health information to bill and collect payment for services. This may include sharing information with health plans, billing services, clearinghouses, or other parties involved in payment and claims processing.
Health care operations
Medtriq may use and share health information for practice operations, such as quality review, care coordination, staff training, credentialing, compliance activities, business management, technology support, and other activities needed to operate the practice.
Appointment reminders and communication
Medtriq may use your contact information to communicate about appointments, forms, billing, care coordination, and practice updates. Medtriq may communicate by phone, text, email, patient portal, intake platform, telehealth platform, or other practice-approved systems.
Some communication methods may have privacy or security limitations. Medtriq may direct patients to specific systems or workflows for clinical communication.
Business associates and service providers
Medtriq may share health information with vendors or service providers that help operate the practice, such as billing services, electronic health record systems, secure messaging platforms, telehealth platforms, intake systems, email or website vendors, consultants, and technology providers. These parties are expected to protect health information as required by law and applicable agreements.
Uses and disclosures allowed or required by law
Medtriq may use or share health information when allowed or required by law, including:
- Public health reporting
- Health oversight activities
- Reporting suspected abuse, neglect, or domestic violence when required or permitted by law
- Responding to court orders, subpoenas, or other lawful processes
- Law enforcement purposes in limited situations
- Workers' compensation or similar programs
- Coroners, medical examiners, or funeral directors when applicable

Substance-use treatment information
Some substance-use treatment records may receive additional privacy protection under federal law, including 42 CFR Part 2, when that law applies. These records may have extra restrictions on use and disclosure. Medtriq follows applicable privacy laws for substance-use treatment information.
When written authorization may be required
Medtriq will generally ask for written authorization before using or sharing health information for purposes not described in this notice. Written authorization may be required for certain disclosures of psychotherapy notes, certain marketing activities, and certain sales of protected health information.
Medtriq does not sell patient health information.
Your rights
You have rights regarding your health information. These rights may be subject to legal limits, verification requirements, and reasonable processing time.
Right to inspect and receive a copy
You may ask to inspect or receive a copy of certain health information Medtriq maintains about you. Medtriq may provide records electronically or on paper, depending on the request and applicable law.
Right to request correction
You may ask Medtriq to correct health information you believe is inaccurate or incomplete. Medtriq may deny the request in some circumstances, but you may have the right to submit a written statement of disagreement.
Right to request confidential communications
You may ask Medtriq to contact you in a specific way or at a specific location. Medtriq will consider reasonable requests.
Right to request limits
You may ask Medtriq not to use or share certain health information for treatment, payment, or health care operations. Medtriq is not required to agree to every request, except in certain situations required by law.
Right to an accounting of disclosures
You may ask for a list of certain disclosures Medtriq made of your health information. This list does not include every type of disclosure, such as many disclosures for treatment, payment, or health care operations.
Right to a paper copy
You may ask for a paper copy of this notice at any time, even if you have agreed to receive it electronically.
Right to choose someone to act for you
If you have given someone medical power of attorney, or if someone is your legal guardian, that person may be able to exercise your rights and make choices about your health information.
Complaints
If you believe your privacy rights have been violated, you may contact Medtriq.
Medtriq will not retaliate against you for filing a complaint.
You may also file a complaint with the U.S. Department of Health and Human Services Office for Civil Rights.
Changes to this notice
Medtriq may change this notice from time to time. The revised notice will apply to health information Medtriq already has and to information Medtriq receives in the future. The current notice will be posted on this website.
